Hospice LVNs & RNs needed

As a Licensed Practical Nurse (LPN),  Registered Nurse (RN), you will play a vital role in delivering compassionate and high-quality patient care. Your expertise will shine as you assist in the assessment, planning, and implementation of nursing care for hospice. What you’ll do

• Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
• Collaborate with physician and RN to implement the patient's care plan.
• Implements the IDG nursing care plan for each patient.
• Maintain accurate medical documentation and records in compliance with HIPAA regulations.
• Support patients and their families by providing education on health management and medication administration.
• Conduct physical examinations and assessments to identify patient needs and report changes to the supervising nurse or physician.
• Maintains accurate and complete records of observations, treatments and care of patients.
• Participate in ongoing training and professional development to enhance your nursing skills.

Basic qualifications

• Valid LPN license in the state of practice.
• Graduate of a state approved school of practical nursing.
• Minimum of 1 year experience in nursing, preferred.
• Acceptance of philosophy and goals of this Hospice.
• Ability to exercise initiative and independent judgment.
• Completion of a Hospice training program prior to providing care.
