RN Case Manager

Key Responsibilities:

• Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
• Provide direct nursing care and support, including medication administration and monitoring.
• Collaborate with interdisciplinary teams to ensure optimal outcomes for individuals.
• Maintain accurate and up-to-date medical records.

Qualifications:

• Active nursing license in North Dakota.
• Strong assessment skills and organizational abilities.
• Experience in intellectual disabilities, mental health, or behavioral health (preferred).
• A compassionate and empathetic approach to care.
