Job Responsibilities:

• Conduct thorough patient assessments and develop individualized care plans.
  
  
• Deliver skilled nursing care according to physician orders and care protocols.
  
  
• Educate patients and families on health maintenance, disease prevention, and care instructions.
  
  
• Maintain accurate documentation in compliance with agency and regulatory standards.
  
  
• Collaborate with the BrightStar Care team and other healthcare professionals to ensure optimal patient outcomes.
  
  

Qualifications:

• Active RN license in the state of practice.
  
  
• Previous experience in home health or acute care settings is preferred.
  
  
• Strong clinical, organizational, and communication skills.
  
  
• A passion for providing compassionate, high-quality care.
