Home Health Clinical Support RN - Multiple Locations!!

As a Home Health Clinical Support RN, you will provide crucial clinical oversight, assessment, and intervention for home-based patients with a wide range of care needs. You will help coordinate services, conduct regular patient evaluations, and ensure adherence to regulatory compliance and company protocols. This role offers a blend of direct patient engagement and collaborative support for nursing field staff in various home care settings.

Key Responsibilities: Home Health Nursing Assessment, Education & Team Collaboration

• Provide all levels of care to the patients
• Support patients in Evanston, IL and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule
• Demonstrate experience with tablets, mobile phones, and EMR software

Qualifications: Registered Nurse License & Home Health Clinical Competency

• Current, unrestricted Registered Nurse (RN) license in the state of service.
• High School/GED
• Epic Home Health (1+ years)
• 2+ years of experience in ANY of the following: Home Health Care and Home Health
• 1+ years of experience in ANY of the following: Electronic Medical Record (EMR), Other EMR and TruChart EHR
• Valid Registered Nurse (RN) in the state of Illinois (IL)
• Valid Regular Driver License (DL)

Schedule and Work Locations: Flexible RN Jobs in Multiple Areas

• Monday through Friday on the day shift; no weekends, on-call or holidays
