An Assisted Living and Memory Care Community

We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in a part time or as needed, daytime position at our memory care community. This role is essential in delivering high-quality nursing care aligned with each resident’s individual assessment and care plan, under the guidance of the Wellness Director.

• Deliver routine daily nursing care and services to residents.

• Perform vital sign checks and health assessments.

• Coordinate with CNAs to collect information on resident conditions and needs.

• Administer medications and ensure accurate documentation.

• Provide clinical supervision and leadership to care staff.

• Support a positive, resident-focused environment.

• Other duties as assigned by the Executive Director

• Current Illinois RN or LPN license in good standing.

• Recent experience in a Memory Care community

• Strong clinical, assessment, and supervisory skills.

• Familiarity with medication administration and care documentation.

• High school diploma or GED.

• Completion of a state-approved nursing assistant program.

• Active status on the IDPH Health Care Worker Registry.

• Ability to lift up to 25 lbs and perform physical tasks typical of CNA support.

• Valid CPR certification (recently expired accepted; recertification available).
