Currently seeking an experienced IDD Program Specialist to be responsible for the monitoring and reporting for functions related to IDD program participants' through the Office of Developmental Program. Job responsibilities include but are not limited to:
- Intake and enrollment of new participants
- Assist in training/mentoring of allDirect Support Professionals
- Attending participant meetings, including per participant: two annual ISP meetings, ISP critical revision meetings as needed, and behavioral support plan meetings as needed
- Prepare and submit monthly progress reports for each IDD participant.
- Prepare and submit biennial and annual progress reports for each IDD participant.
- Review participant logs monthly or more frequently as necessary.
- Track and notate participants' progress towards ISP goals.
- Evaluate and determine participant's service needs for progress towards goals, i.e. DSP services or community integration services.
- Review each participant's ISP with all direct care workers prior to that DCW serving that participant, including after a change in ISP. Ensure such review is properly documented.
- Train direct care workers in IDD program requirements and the performance of the DCW job duties under the ID programs
- Assisting the supervisor of direct care workers to hold DSP staff accountable for documentation, progress on outcomes, etc.
- Assist with self-audits, with preparation for state audits and during state audits
- Conduct Certified Investigations and reports
- Participate in supervisory on-call rotation
Requirements:
- Bachelor's Degree in related field preferred
- Ability to pass background checks
- Satisfactory driver's record check, valid driver's license, and reliable vehicle
- Obtain and maintain Office of Developmental Program Certified Investigator certification
- Be at least 18 years of age and legally able to work in the United States
- Be able to attend program-related meetings, including those outside the normal M-F, 8:30am-5pm work schedule, including evenings or weekends.
