Full-Time Home Health Admissions RNSchedule: Saturday, Sunday, Monday (Full-Time)

Due to continued growth throughout our Massachusetts market, we are expanding our clinical team and seeking an experienced Home Health Admissions Registered Nurse to help meet the increasing demand for home health services across Greater Boston and the North Shore region.
As an Admissions RN, you will play a critical role in creating exceptional first impressions for our patients and families. You will serve as the clinical bridge between referral and care delivery, completing comprehensive start-of-care assessments, establishing individualized care plans, and ensuring patients begin their home health journey with confidence and support.

Complete Start of Care (SOC) visits and comprehensive nursing assessments for new home health patients
Evaluate patients' clinical, functional, psychosocial, and environmental needs
Develop individualized plans of care in collaboration with physicians and interdisciplinary team members
Accurately document assessments and admission information in the electronic medical record
Provide patient and caregiver education regarding diagnoses, medications, treatments, and home safety
Identify patient risks and coordinate appropriate interventions to support successful outcomes
Communicate significant findings and changes in patient condition to physicians and clinical leadership promptly
Ensure compliance with agency policies, state and federal regulations, and quality standards
Support continuity of care through effective collaboration with case managers and branch clinical teams
Maintain patient confidentiality and uphold HIPAA regulations at all times
Current, unrestricted Massachusetts Registered Nurse (RN) license
Minimum of one (1) year of nursing experience required
Previous Home Health experience strongly preferred
Prior admissions or Start of Care experience preferred
Experience with HomeCare HomeBase (HCHB) preferred
Associate Degree in Nursing required; BSN preferred
Strong assessment, clinical decision-making, and documentation skills
Ability to work independently while managing a dynamic daily schedule
Valid driver's license, reliable transportation, and current auto insurance
Willingness to travel throughout the assigned coverage area
