Care Team Manager – Home Care
Location: Grand Rapids, MI
Schedule: Full-time time Monday through Friday, with occasional on-call evenings and weekends
Pay: 20.00 to 24.00 per hour, depending on experience
Start Date: Negotiable

Position Summary
We are seeking a dependable and proactive Care Team Manager to oversee caregiver staffing, client scheduling, and daily care operations. This role requires strong communication, problem-solving solving and the ability to respond quickly to caregiver call-outs. The Care Team Manager will also serve in a rotating on-call role for evenings and weekends and must be willing to step into client visits when a caregiver is not available.
Key Responsibilities
• Schedule caregivers to manage call-outs and ensure all client shifts are covered
• Provide on-call support for urgent staffing or client needs during evenings and weekends on rotation
• Step in for client visits when necessary to maintain care continuity
• Support mentor and supervise caregivers with professionalism and accountability
• Communicate with families regarding schedules, care updates, and concerns
• Assist with caregiver onboarding orientation and performance follow-up
• Track schedules, timesheets, and maintain accurate care records
• Assist with new client inquiries and coordinate in-home assessments
Qualifications
• Experience in memory care, assisted living, home care, or caregiver leadership preferred
• CNA caregiver or clinical background strongly preferred
• Comfortable providing hands-on care when needed to support clients
• Strong organizational skills and ability to manage frequent changes calmly
• Clear professional communication with caregivers, clients, and families
• Comfortable with technology and scheduling platforms WellSky is a plus
