Job Description
Job Title: Wraparound Care Coordinator – CSED Waiver
Job Summary:
The Wraparound Care Coordinator provides individualized care coordination for children and families under the CSED Waiver program, guided by the wraparound philosophy that emphasizes empowering families. Wraparound care is a structured process — not a single service — designed to coordinate formal services and natural supports to achieve positive outcomes for youth and their families. Flex Hours Available. Remote work may be available.
Key Responsibilities:
Build and maintain a Child and Family Team for each youth, including the family, advocates, lead agency staff, providers, and informal supports.
Initiate contact with families within 3 days of referral and conduct face-to-face meetings within 7 business days to discuss goals, roles, expectations, safety, and stability needs.
Develop and maintain individualized Plans of Care, incorporating formal and informal supports and evidence-based practices.
Create a Family Narrative outlining the family’s story, vision, needs, and strengths.
Develop and update Crisis/Safety Plans and Natural Disaster Plans, ensuring 24/7 availability for family support during crises.
Maintain ongoing knowledge of community resources, develop relationships with service providers, and identify supports that strengthen the family’s capacity and independence.
Evaluate Plan of Care effectiveness regularly and collaborate with the Child and Family Team to adjust interventions as needed.
Maintain a caseload not exceeding 15 partnered families.

Education & Experience:
Minimum B.A. or B.S. degree in Social Work, Psychology, Human Services, or a related field.
Prior experience in human services required; experience with Wraparound and CSED Waiver services preferred.
Must attend all required trainings to maintain and advance knowledge of Wraparound and CSED Waiver services.
Qualifications:
Strong interpersonal, communication, and organizational skills.
Ability to coordinate multiple providers and supports across systems.
