RN / LCSW Case Manager

A-Line is hiring Full-Time Registerer Nurse Case Managers - LTC with a Major Health Plan company based on Charlotte, NC.

**This is a Hybrid position doing visits in LTC facilities with some remote days! The RN or LCSW must be based near or in Charlotte, NC or Rocky Mount, NC**

Job Title: Hybrid Registered Nurse Care Manager

Hourly Rate: $40.00-42.00 PLUS Mileage reimbursement & Lodging reimbursement if needed
Work Schedule: Monday–Friday, 8 AM – 5 PM EST

Work Location:

• Remote with travel - field-based with 1–2 remote administrative days weekly
• Locations:
• Rocky Mount, NC – 1 opening
• Charlotte, NC –1 opening RN only
• Note: Candidate must reside within a 2.5-2.75 hour radius one way of assigned region for member visits.
• Travel: Approximately 80% field-based; valid driver’s license required. Mileage and lodging reimbursed.
• 2-3 visits per day 3-4 days a week; 1-2 days remote
