About the Role:
The LPN Clinical Team Lead plays a crucial role in enhancing patient care by collaborating with a team of care navigators who assist patients in navigating the healthcare system. This position is responsible for ensuring that patients receive timely and appropriate services, thereby improving health outcomes and patient satisfaction. By fostering a collaborative environment, the Team Lead will empower team members to provide exceptional support to patients and their families. In addition, the Team Lead will serve as a float care navigator to ensure adequate account coverage at all times. Ultimately, this role aims to create a seamless healthcare experience that promotes wellness, effective management of health conditions, and a positive teamwork environment.
Minimum Qualifications:
- Associate's degree in nursing.
- At least 3 years of experience in care coordination or case management within a healthcare setting.
- Strong understanding of healthcare systems, resources, and patient advocacy.
- Proficient in multiple electronic health records (EHR) systems and care management software.
- Experience in a leadership or supervisory role.
Preferred Qualifications:
- Bachelor's degree in a relevant field.
- CCM and RPM experience.
Responsibilities:
- Serve as a Nurse Navigator to a designated load of patients.
- Collaborate with healthcare providers, community organizations, and other stakeholders to facilitate comprehensive patient care.
- Achieve organizational performance standards.
- Mentor a team of care navigators, providing guidance in their daily activities through task escalation support.
- Provide account coverage on a regular basis and as needed.
Skills:
The required skills for this position include proficiency in navigating multiple EHRs, which are essential for serving across various accounts to support the care navigation team. Excellent communication skills are vital for effectively interacting with patients, families, healthcare providers, and the Visium team to ensure a cohesive care experience. Problem-solving skills will be utilized daily to address patient needs and overcome barriers to care. Additionally, organizational skills are necessary for prioritizing escalations and coordinating various services efficiently.
