Job Description
POSITION OVERVIEW
Works on a multidisciplinary team in a patient centered care setting providing home healthcare to Native
Americans. This position will be an intricate part in achieving the departments visions, mission and
program objectives, ensuring compliance with home health requirements and providing senior management, leadership and healthcare expertise. Collaborating with physicians within and outside our
clinics on patient cares throughout the community health service area.
PERFORMANCE-BASED JOB FUNCTIONS AND OBJECTIVES
1. Provide direct patient care within established protocols that result in timely billing each month, such
as:
- a. Developing treatment plans;
- b. Ordering appropriate diagnostic and screening evaluations;
- c. Referring for specialized consultations, when necessary or collaborating with
- multidisciplinary team and making specialist referrals when needed;
- d. Providing education and counseling to the patient and family in preventive care, medical
- conditions, health lifestyle habits, and use of prescribed treatments and medication;
- e. Recognizing limitations and seeking medical discussion and advice with physician and other
- health professionals when necessary; and,
- f. Documenting all patient services in the electronic medical records.
2. Manages and administers medications by coordinating medication orders with the pharmacy and
other providers, completing medication refills as indicated and assessing for medication
compliance.
3. Obtain and review outside medical records and information from outside sources as appropriate
and indicated and document relevant information in the medical record.
4. Provide consultation and coordination to other health staff within and outside of the division for the
purpose of providing comprehensive services.
5. Assist in development and implementation of in-service educational programs and follow-up to
evaluate effectiveness of the programs to determine the need for revision of methods and
materials.
6. Develop strategic plans and assists with data collection, reporting and analyzation of data to help
drive effective and efficient programs.
7. Assist with monitoring and overseeing budget activity including recommending modifications when
needed; approved purchase request and expenditures on a monthly basis.
8. Complete necessary annual continuing education to meet state practice and certification
requirements and avoid any suspensions to practice in a year.
9. Comply with applicable state, federal and Ho-Chunk legal and professional requirements as a
practicing Family Nurse Practitioner. Delegate duties to allied staff as appropriate for education and
expertise in order to actualize performance of duties relevant to their scope of work as nursing and
ancillary support staff and result in attainable patient visit counts per month.
10. Perform other duties as assigned by supervisor within the scope of this job description which may
include but not limited to the following:
- a. Attend meetings;
- b. Supervise staff
JOB RESPONSIBILITY

Freedom to Act: Sets broad policies and objectives; Operates with significant independence; Subject to general input from supervisor; Subject to regular review by supervisor
MINIMUM QUALIFICATIONS
EDUCATION:
1. Possess a Master’s Degree in Nursing (MSN) or Doctor of Nursing (DNP) degree.
2. Nurse Practitioner, family practice preferred, certified from an accredited governing body.
ESSENTIAL:
1. Current valid Registered Nurse license for the State of Wisconsin in good standing.
2. Possess a State of Wisconsin Advanced Practice Nurse Prescriber.
3. Posses or be eligible to posses a controlled substance registration certification from the
Drug Enforcement Agency.
4. Possess and maintains current BLS and ACLS certification.
5. Must maintain patient confidentiality in accordance with the Privacy Act/HIPAA.
6. Possess a valid driver’s license, dependable transportation and proper insurance.
EXPERIENCE:
1. Previous supervisory experience in home health setting preferred.
2. Minimum five-year experience as a Registered Nurse; preferably in-home care setting.
3. Preferred working knowledge and contracting experience with Managed Care
Organizations.
4. Possess advanced computer skills and has experience with using electronic health records.
5. Experience with quality improvement to assist with remaining in compliance with State and
Federal licensing standards.

8. Knowledge of home health nursing practices and principles.
9. Knowledge and understanding of budget process, grants and reporting system.
10. Knowledge and respect of Ho-Chunk culture, traditions, and community concerns and ability to
work with a diverse population.
11. Ability to lead and promote a healthy, non-abusive lifestyle.

WORKING CONDITIONS
1. General office setting in the department facilities as well as community sites.
2. Work performed in the community is sometimes subject to conditions ranging from inclement
weather to dangerous conditions such as snow/ice, cold, heat, noise, wetness/humidity, vibration,
sudden temperature changes, and poor illumination at the job site or die to travel on the job. Travel
to and from field locations may subject worker to increased risk of driving hazards. Community
locations may subject worker to communicable diseases, insects and other disease vectors, toxins,
hazardous materials and animals.
3. In all settings, employees in this classification may occasionally need to relate to members of the
public who exhibit challenging, atypical or hostile behaviors and/or communication.
4. Traveling is a necessity.
5. Potential exposure to hazardous and bio-hazardous material.
