Salary Range: $60-80/Hr DOE
Job Summary:
The Per Diem / On-Call Pharmacist provides coverage for pharmacy operations as needed to ensure continuity of patient care during staff absences (e.g., illness, vacation) and high-volume periods. This position dispenses medications prescribed by authorized prescribers and provides medication counseling to patients. The pharmacist advises providers and clinical staff on drug selection, dosage, interactions, contraindications, and side effects, consistent with ambulatory pharmacy practice at Mattawa Community Medical Clinic.
Supervisory Responsibilities:
Supports daily pharmacy operations when scheduled. May assume temporary leadership of workflow in the absence of the pharmacist-in-charge to maintain safe and efficient operations. Does not carry ongoing responsibilities for hiring or performance management.
Duties/Responsibilities:
- Reviews prescriptions to assure accuracy, ascertain needed ingredients, and evaluate clinical suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper storage.
- Assesses the identity, strength, and purity of medications. Compounds and dispenses medications as prescribed or oversees these activities.
- Provides patient counseling and specialized services to help manage conditions (e.g., diabetes, asthma, smoking cessation, hypertension).
- Collaborates with clinic providers and staff to plan, monitor, review, and evaluate the quality and effectiveness of medication therapy; analyzes prescribing trends to support patient safety and adherence.
- Maintains records such as pharmacy files, patient profiles, inventory logs, and controlled-substance documentation in accordance with federal and Washington State regulations.
- Plans, implements, and maintains procedures for mixing, packaging, labeling, storage, security, and proper disposal of pharmaceuticals.
- Orders and purchases pharmaceutical supplies, ensuring proper storage, handling, and stock rotation.
- Provides on-call or scheduled coverage during pharmacist absences due to illness, vacation, trainings, or emergencies; responds to short-notice requests when feasible to maintain uninterrupted services.
Required Skills/Abilities:
- Strong patient service orientation and clear, compassionate communication.
- Expert knowledge of pharmacology and drug interactions; sound clinical judgment in an ambulatory setting.
- Meticulous attention to detail with the ability to balance competing priorities and maintain accuracy under time pressure.
- Familiarity with EHR and pharmacy information systems; ability to document and retrieve information efficiently.
- Flexibility to accept variable scheduling, including short-notice, weekend, or holiday coverage as needed.
Education and Experience:
- Doctor of Pharmacy (Pharm.D.) degree required.
- Active and unrestricted Washington State Pharmacist License required.
- Prior experience in ambulatory or community pharmacy preferred.
Physical Requirements:
- Prolonged periods standing, bending, and working at a computer.
- Ability to lift up to 15 pounds occasionally.
