In this Long-Term Care LPN role, you’ll oversee essential nursing duties while nurturing residents’ overall well-being. Key responsibilities include monitoring patient conditions, administering prescribed medications within scope of practice, assisting with activities of daily living, wound care, and preventive care, along with timely documentation, accurate charting, and communication with interdisciplinary teams. You’ll develop and implement care plans, observe and report changes in patient status, and participate in family conferences to align on goals and expectations.
