Job Description
Schedule: Full Time – 80 hours/pay period – Monday to Friday – 8 a.m. to 4:30 p.m.
As a Manager of Home Health Clinical Operations, you will oversee the day-to-day clinical provision of home health services, ensuring high-quality, client-centered care that aligns with state, federal, and Vivie standards. You will lead an interdisciplinary team, optimize operational efficiency, and drive excellence in home health services while fostering a supportive and compliant care environment.

As a Manager of Home Health Clinical Operations, you will:
- Lead Clinical Operations – Oversee the interdisciplinary team to ensure timely, appropriate, and high-quality care while maintaining regulatory and billing compliance.
- Supervise and Support Staff – Manage field and office staff to optimize care delivery, meet productivity standards, and maintain best-practice team utilization.
- Ensure Efficient Case Management – Assign caseloads based on client acuity, staff availability, and performance benchmarks to balance workload and quality outcomes.
- Drive Quality and Compliance – Partner with Quality and Compliance teams to analyze clinical outcomes, CAHPS scores, Home Health Compare data, and PEPPER reports, implementing action plans for improvement.
- Foster Growth and Retention – Collaborate with HR, Training, and Quality teams to provide strong onboarding, ongoing education, and staff development for optimal team retention and clinical excellence.

This job also requires:
- Current RN, PT, OT, or Speech Language Pathologist license in the state of Minnesota.
- Bachelor’s degree preferred.
- Minimum 1 year of home health experience preferred.
- Current CPR certification.
- Strong leadership, team management, and clinical oversight skills.
- Knowledge of Medicare, Medicaid, and home health regulations.
- Strong communication, collaboration, and problem-solving skills.
- Valid driver’s license and access to an insured personal vehicle.
- Ability to pass state mandated background check.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with staff, clients, and referral sources.
