MDS Coordinator

Courtyards Community Living Center is looking for a MDS Coordinator to join our growing team!

MDS Coordinator duties include but are not limited to:

• Maintain and update schedules of MDS's that are due
• Coordinate the work of the Interdisciplinary Team to complete the MDS, which includes the CAAs and care plan
• Enter or supervise input of MDS into computer systems
• Act as the leader of the MDS and care plan meetings
• Coordinate the completion of the care plan
• Other tasks as required

Requirements:

• Registered Nurse ( RN ) license in good standing required
