Opportunity:
RN - Home Health
Job Details:
• Work 8-hour day shifts, approximately 8:00 AM to 4:30 PM with possible flexibility based on census
• 40 hours per week guaranteed
• Possible weekend and holiday call responsibilities
• Weekday on-call duties - can be low censused and placed on call with 2-hour minimum callback
• Travel 25-60 miles per day on average, up to and over 100 miles due to large territory coverage
• See 20-25 patients weekly with 4-5 visits per day on average
• Mileage reimbursement at current government rate
• Required certifications: AHA BLS, active Michigan and Wisconsin RN licenses, valid driver's license
• OASIS certification is a plus
• Minimum 2+ years home health experience preferred with travel experience required
• Must have experience with OASIS and Epic charting systems
• Perform OASIS admissions, episode management, doctor calls, and care coordination
• Complete timely documentation in EMR and maintain clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
• Associate degree or certificate required, bachelor's degree preferred
• Must demonstrate strong time management, resilience, dependability, and compassionate patient care
• Eligible for 36 hours of unbillable low census hours during contract
• Mask wearing required at all times during flu season if flu and COVID vaccinations are declined
