About the Role:

As a Licensed Practical Nurse (LPN), you will play a crucial role in providing high-quality patient care. Your primary responsibility will be to support the health and well-being of residents by administering medications, performing wound care, and assisting with daily living activities. You will also be responsible for educating patients and their families about health management and wellness strategies. Collaboration with a multidisciplinary team, including Nurse Practitioners, will be essential to ensure comprehensive care plans are implemented effectively. Ultimately, your work will significantly impact the quality of life for residents, promoting their independence and comfort.

Minimum Qualifications:

  • Valid Nursing License in the state of practice.
  • Completion of an accredited Practical Nursing program.
  • Minimum 3+ years clinical experience in an acute setting or equivalent experience.
  • Nephrology and/or ambulatory experience preferred.
  • Valid CPR certification required.
  • Basic computer skills.
  • High integrity, collaborator with patient service focus and flexibility.
  • Demonstrated ability to take initiative and work independently, yet also function in a highly collaborative, fast paced, team environment.
  • Excellent verbal and written communications skills.

Responsibilities:

  • Provide ongoing patient care utilizing LPN knowledge
  • Maintains patient charts accurately utilizing the Acumen Epic EHR system for patient documentation
  • Prepares for clinics with any physicians within the practice
  • Orders labs, diagnostic testing, and completes referrals to other physicians, as ordered by NANI physicians.
  • Assists office staff with triaging patient clinical phone calls, verifying patient insurance during check-in, and scheduling appointments.
  • Manages critical lab values and follows-up with patients and providers as appropriate
  • Provides patient education
  • Other duties as assigned.
  • Will work 3 days a week.
