We are seeking an upbeat, highly organized, and detail-oriented Patient Care Coordinator to support our patient intake workflow and overall care process. This role combines strong administrative accuracy with patient-facing communication. The ideal candidate ensures nothing falls through the cracks, guides patients through their journey, and collaborates with internal teams to maintain smooth operations, with a patient experience focus.

Key Responsibilities

Patient Coordination & Support

  • Oversee the patient intake process (patients typically initiate it, but you ensure it stays on track and may initiate at times).
  • Serve as the main point of contact for patients via phone, and text with a warm, service-oriented approach.
  • Coordinate with the Medical Records team as needed for seamless handoffs and ongoing communication.
  • Track patient progress in dashboards, spreadsheets, and company systems; proactively identify and address gaps.
  • Scheduling of consultation for the patient with the appropriate physician.
  • Follow up with patients after physician consultations to confirm next steps.

Administrative & Data Management

  • Maintain and update dashboards, trackers, MD Direct App, and other documentation with accuracy.
  • Perform regular data entry, ensuring completeness and correct documentation.
  • Review Fax Plus and other medical systems for medical records, imaging, etc., and upload appropriately.
  • Document patient interactions and status changes in the appropriate systems.
  • Escalate issues or delays in patient progression to internal teams as needed.

Cross-Team Collaboration & Process Improvement

  • Communicate clearly and consistently with internal teams to maintain visibility of patient status.
  • Assist in identifying process improvements to streamline the patient journey.

  • Self-Starter. Takes initiative for tasks and does not need a lot of direction.
  • Exceptional attention to detail; ensures nothing slips through the cracks.
  • Strong verbal communication skills; comfortable and friendly on the phone.
  • Highly organized; able to manage multiple patients simultaneously.
  • Customer-first mindset with a service-oriented approach.
  • Tech-savvy: comfortable with spreadsheets, dashboards, EHR-like systems, and task tools.
  • Process-driven and proactive; anticipates issues and addresses them before they escalate.
  • Experience in healthcare coordination, administrative support, or patient services is a plus.
