Job Summary
Reporting to the Clinic Manager, the Care Manager coordinates the care and services of CIBD patients across the continuum of illness; promotes and monitors effective utilization of health care resources; and, assumes a leadership role with the multi-disciplinary care team to achieve optimal clinical and resource outcomes. As part of the Case Management Team, the Care Manager provides Team Center Care in a collaborative setting in accordance with the standards and regulations for California Childrens Services (CCS), Genetically Handicapped Persons Program (GHPP), the Joint Commission, and all other State and Federally sponsored programs.

Essential Duties
The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non-profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with Hemophilia, Sickle-Cell Anemia, Thalassemia, Von Willebrand Disease, Mast Cell Activation Syndrome (MCAS) and other related benign hematology conditions. CIBD is the regional grantee for the Western States Region for Hemophilia, the Pacific Sickle Cell Regional Collaborative (PSCRC) and Networking California for Sickle Cell Care (NCSCC) providing regional leadership, fiscal and administrative oversight, grants management, capacity building, clinical education, technical assistance, evaluation, and outreach to public and private health agencies at the local, state, and federal levels.

Essential Duties
- Patient Coordination: Collaborate with patients to identify their needs and understanding of the care plan. Coordinate their care with external facilities, including home health agencies, third-party payers, pharmacies, and lab facilities.
- Effective Collaboration: Work closely with internal partners (e.g., physicians, nurses, authorization coordinators) and external providers (e.g., home health agencies, pharmacies, lab facilities) to ensure seamless coordination of care and fulfillment of orders from initiation to completion.
- Self-Management and Communication: Exhibit self-direction while effectively working with others. Demonstrate strong organizational and documentation skills. Employ effective written and verbal communication, utilizing strong computer skills for navigating the EMR, submitting electronic prescriptions, sending electronic approval requests, and engaging through email, Zoom, and Microsoft Teams. Use critical thinking and problem-solving capabilities to address conflicts and resolve issues while providing patient education.
- Team Support: Promote a comprehensive care team model and be prepared to become cross-trained as necessary for staffing support. Maintain strong communication to foster positive interpersonal relationships within the team.
- Program Development: Enhance the established program as it relates to the Care Managers assigned patient population through community outreach, engagement with school and community-based organization liaisons, active participation in committees, conference attendance, and additional duties.
- Graduate from an accredited Registered Nurse (RN) Program
- Bachelor of Science in Nursing (BSN)
- Active California State Registered Nurse (RN) License
- Minimum 2 years nursing experience in acute care or ambulatory setting
- Basic Life Support (BLS)
- HIPAA training
- Ability to work onsite (not a remote role)
- Experience in Case Management
- Knowledge of CCS/GHPP/Medi-Cal regulations and requirements
- Knowledge of third party payer methods and regulations
- Experience in hematology/oncology nursing in an adult or pediatric setting
- Bilingual English/Spanish
