Position Overview

We are currently seeking an experienced and compassionate Nurse Practitioner (APRN) or Physician Assistant (PA) to join our Adult Medicine clinic. In this vital role, you will provide comprehensive primary care services with an emphasis on health promotion, disease prevention, and chronic disease management. You?ll work collaboratively with department physicians and provide patient referrals when specialized diagnostic or therapeutic care is required.

This position is eligible for participation in the National Health Service Corps (NHSC) Loan Repayment Program.

Key Responsibilities

  • Deliver high-quality, evidence-based primary care to adult patients
  • Conduct comprehensive assessments, formulate diagnoses, and implement treatment plans
  • Collaborate with physicians and care teams to ensure continuity of care
  • Provide culturally competent care to a diverse patient population
  • Educate patients on health conditions, preventive care, and treatment options
  • Maintain accurate and timely documentation using the electronic health record (EHR)
  • Contribute to departmental quality measures and clinical outcomes
  • Participate in organizational initiatives and team-based care delivery

Required Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Valid APRN or PA license in the State of Connecticut (Required)
  • BLS certification (Required)
  • Eligible for or possess DEA and Connecticut Controlled Substance Registration
  • Strong clinical decision-making and diagnostic skills
  • Excellent interpersonal and written communication abilities
  • Commitment to serving medically underserved populations

Preferred Qualifications

  • 1?2 years of experience in an ambulatory or urgent care setting
  • Proficiency in electronic health record systems
  • Spanish language skills (Preferred)
  • Knowledge of and experience with quality improvement or value-based care initiatives
