Licensed Practical Nurse (LPN) Adult Day Program
Location: Stockbridge, Georgia
Schedule: MondayFriday, 8:00 AM 3:00 PM

POSITION SUMMARY
The Licensed Practical Nurse (LPN) provides nursing care and clinical oversight to individuals enrolled in the Adult Day Program, in accordance with the Plan of Care, DBHDD Community Standards, DCH/Medicaid regulations, HFRD guidelines, and under supervision of the Registered Nurse (RN)/Clinical Director. The LPN ensures the health, safety, dignity, and well-being of each client while upholding A New Well, Inc.s mission, philosophy, policies, and procedures.
ESSENTIAL DUTIES & RESPONSIBILITIES
(In accordance with DBHDD & DCH requirements)
Clinical Care & Health Services
The LPN may provide the following services, as permitted by Georgia LPN scope of practice and the Plan of Care:
- Gastrostomy Care (G-Tube/J-Tube): maintenance, feeding, and medication administration
- Specialized wound care and dressing changes
- Medication administration, monitoring, injections, and documentation
- Tube feedings (bolus or continuous)
- Measuring intake/output and hydration monitoring
- Vital signs and routine health assessments
- Nebulizer treatments / MDI inhaler administration
- Oxygen administration via tank or concentrator
- Ventilator alarm checks and functional monitoring (as permitted in non-invasive settings)
- Catheter care and monitoring
- Ostomy/stoma care, bag changes
- Specimen collection for lab analysis
- Observing, identifying, and reporting changes in condition
- Monitoring for adverse reactions, side effects, and medication tolerance
- Supporting ADLs within the scope of LPN practice as needed
Regulatory & Documentation Responsibilities
- Complete electronic progress notes that meet DBHDD, DCH/Medicaid, and agency documentation standards
- Conduct clinical record reviews as assigned
- Document supervisory observations, medication administration, and skilled nursing tasks
- Communicate all pertinent changes in a clients condition to the RN/Clinical Director immediately
- Ensure services delivered match the Individual Service Plan / Plan of Care
- Maintain compliance with DBHDD incident reporting procedures when applicable
- Promote HIPAA confidentiality and secure handling of protected health information
- Ensure all clinical actions align with Georgia Board of Nursing scope requirements
Person-Centered & Family Support
- Provide education to the client and/or responsible party about medical care, treatment plans, and health management
- Respect cultural, personal, and religious needs
- Communicate effectively and professionally with families, caregivers, and interdisciplinary team members
Quality, Safety, & Compliance
- Perform all client care with an emphasis on safety, infection control, and error prevention
- Participate in interdisciplinary team conferences and care reviews
- Maintain professional competency through continuing education
- Support emergency preparedness protocols (DBHDD & internal)
- Follow Adult Day Program rules, RN directives, and agency safety guidelines
SKILLS & QUALIFICATIONS
- Current Georgia LPN license in good standing
- Current CPR/First Aid certification
- Strong interpersonal and customer service skills
- High level of patience, empathy, and professionalism
- Excellent communication: verbal, written, and electronic documentation
- Ability to work effectively in a team environment
- Strong clinical judgment and problem-solving skills
- Detailed understanding of DBHDD & DCH service standards
- Ability to lift, transfer, or support clients as needed
- Comfortable working with elderly adults and individuals with developmental disabilities or chronic medical needs
