Job Description
Under the direction of the Family Support Supervisor, the Family Support and Training Partner is responsible for providing Peer to Peer support- a service that involves a structured, strength-based relationship between a Family Partner and a parent/caregiver. The purpose of this service is to assist the family in improving a youth’s emotional and behavioral needs by strengthening the parent/caregiver capacity to parent the youth. The intention of the service is to improve the youth’s functioning in the community and at home.
Experience as a caregiver of a youth with special needs and preferably a youth with mental health needs required. Bachelor’s Degree in human services related field preferred; Associates degree in human services related field or a minimum of two years experience working with youth/families may be substituted. Bilingual, Bicultural preferred. Spanish preferred or other non-English language capability considered.
Strong relationship-building skills required. Excellent communication and computer skills required. Valid driver’s license, reliable and safe transportation required. Work schedule must accommodate the needs of the participants and program delivery schedule.

Requirements:Purpose:
The CFSS Family Support Partner provides strength-based supports to individuals and their families to support them in the community and to assist with their specialized needs.
Responsibilities Include:
- The Family Partner provides hope, information, encouragement, and one-to-one support to parents/caregivers of children with emotional/behavioral health issues.
- Assist in the creation and maintenance of a positive and collaborative working environment where all employees, enrolled youth and their families feel welcome, supported and comfortable.
- All activities described will be delivered at the CSA offices, in the community, or in the family’s home depending on the preferences of the enrolled youth and family.
- Develop the Strength and Needs document and activity logs as trained in the wrap around process.
- Work to build strong, functional working relationships with the enrolled youth and their families.
- Develop and implement outreach strategies when necessary to enable parents/caregivers of enrolled youth to participate fully in the Wraparound process.
- Assist the Care Coordinator to develop with each youth and family a Care Planning Team composed of family members and their natural supports.
- Serve as a mentor to parents/caregivers to improve their ability to effectively advocate for their youth and navigate the child/family serving human services system.
- Work closely with Care Coordinator, the family, and the Care Planning Team to monitor the Care Plan, and serves as an advocate/liaison for the family when appropriate to access new or different services.
- Participate as appropriate in all meetings, training, evaluations, supervision, quality management, and related activities as directed by the Family Support Supervisor.
- Facilitates and monitors service authorization in collaboration with each family to assure compliance with LUK policies, procedures and annual budget.
- Participate in FCT on-call system.
- Perform other duties as required and necessary.
Knowledge and Experience Required:
- A High School Diploma or equivalent is required
- Lived experience as a parent/primary caregiver of a youth who is/has been involved in the Mental Health System.
- Driving in the community is a requirement of this position; please ensure you have a valid driver’s license and access to a reliable vehicle.
- Knowledge of behavioral health systems and services, DMH, and DCF resources, special education law and services, and other social service resources, and how to access these services
- Bilingual is a plus but not required.

Salary Range and Benefits:
Pay Grade: 11. Salary Range: $45,000 – $57,000. Salary ranges within each pay grade are based on qualifications, skills, and experience.
