Salary Range: $62,700 - $111,600 (based on experience)
Social Worker - Case Manager Opportunity:

• Full-time Permanent hire
• Schedule: Monday-Friday 8a-5p
• $5,000 Signing Bonus
• Social Worker - Case Manager coordinates and oversees patient care plans by collaborating with the healthcare team to ensure efficient, high-quality care and smooth transitions throughout the hospital stay.

Social Worker - Case Manager Qualifications:

• Social Worker - Case Manager m ust have an unrestricted RN license
• Previous experience in an inpatient rehab/hospital setting preferred
• Previous case management experience required
