Job Description
Aperion Care Park Ridge, a skilled nursing and rehabilitation facility in Illinois, is seeking an experienced and detail-oriented MDS Coordinator to join the clinical leadership team. This role is ideal for a nurse who is highly organized, knowledgeable in MDS/PDPM, and committed to ensuring accurate resident assessments and strong clinical outcomes.
Key ResponsibilitiesComplete and manage the full MDS/RAI process in accordance with state, federal, and CMS regulations.
Coordinate interdisciplinary assessment schedules and ensure timely submission.
Oversee care plans and ensure accuracy of clinical documentation.
Collaborate closely with nursing, therapy, and administrative teams to support resident outcomes and reimbursement accuracy.
Participate in quality assurance, survey preparation, and performance improvement initiatives.
Provide education and support to staff regarding MDS and clinical documentation requirements.
Active RN license in Illinois (required).
Prior MDS Coordinator experience in a skilled nursing facility.
Strong knowledge of PDPM, RAI guidelines, and LTC documentation standards.
Excellent communication, assessment, and organizational skills.
Ability to work independently while collaborating with the interdisciplinary team.
