OVERVIEW

The Refugee Wellness Program of USCRI Des Moines provides case management to refugees residing in the U.S. for less than five years who have chronic or critical physical and mental health conditions. The Case Manager will support clients with gaining access to needed medical, social, and other services, while also serving to enhance the capacity of local medical providers to serve refugee communities. The Case Manager will provide intensive case management support, client-centered advocacy, referral to appropriate community support services, and transportation support through a trauma-informed approach. USCRI has a mandatory COVID-19 vaccine policy. All employees must be fully vaccinated to work at any/all locations of USCRI.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Increase access to healthcare resources for refugees;

• Facilitate a coordinated system of health care for refugees in the Des Moines area;

• Build the capacity of physical health providers and other service providers to care for refugee health needs;

• Provide direct case management services to refugees in the community;

• Conduct home visits, intakes and assessments of clients referred to the program;

• Coordinate appointments and services, maintain case files and case notes for clients enrolled in program;

• Track client enrollment, outcomes, and interventions, and provide information in regular reports;

• Work together with team members in the Wellness Program and resettlement agency to serve refugees with medical needs;

• Attend meetings of stakeholders in the health community to create new partnerships and share resources;

• Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary; and

• Perform other job related duties as assigned by supervisor.

POSITION REQUIREMENTS

• Bachelor's degree or equivalent combination of education and work experience in social work, human services, public health, sociology, or a relevant field;

• Prior experience providing case management services to refugees, asylees, and other immigrants preferred;

• Excellent interpersonal skills, including ability to listen, communicate clearly, and build relationships and partnerships with program and Field Office staff;

• Highly adaptable and flexible with the ability to multi-task and prioritize in a demanding work environment;

• Must have a valid state-issued driver's license, automobile insurance, clean driving record and reliable personal transportation;

• Ability to travel within the state of Iowa as required;

• Proficient in Microsoft Office, specifically Word, PowerPoint, and Excel and other database programs as required;

TRAINING REQUIREMENTS

• Satisfactory Completion of USCRI’s Orientation and Training; and

• Complete additional training as identified by supervisor(s), program director or Human Resources

PHYSICAL DEMANDS

• Use of manual dexterity, tactile, visual, and audio acuity;

• Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands;

• Occasional lifting (up to 25 pounds), bending, pulling, and carrying;

Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).
