Requirements:

• Current RN license in the state of practice.
• Minimum of 1–2 years of nursing experience; home health experience preferred.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work independently and manage multiple patient visits efficiently.
• Valid driver’s license and reliable transportation.
• Basic Life Support (BLS) certification required; other certifications may be preferred.

Responsibilities:

• Assess patient health status, develop individualized care plans, and evaluate outcomes.
• Provide nursing interventions including medication administration, wound care, and chronic disease management.
• Educate patients and families on disease management, medications, lifestyle modifications, and self-care techniques.
• Monitor and document patient progress, communicate changes to physicians, and coordinate care with interdisciplinary teams.
• Ensure compliance with infection control, safety protocols, and regulatory standards.
• Support patients and families emotionally and provide guidance on home care resources.
• Participate in quality improvement initiatives and professional development activities.
