Responsibilities:

• Conduct comprehensive evaluations and assessments of patients' physical abilities.
• Develop personalized treatment plans tailored to individual patient needs.
• Implement therapeutic exercises and modalities to enhance recovery.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on rehabilitation techniques and home exercises.
• Collaborate with multidisciplinary teams to ensure holistic care.
• Maintain accurate and timely documentation of patient care and progress.
• Stay updated on best practices and advancements in physical therapy.

Requirements:

• Licensed Physical Therapist in the state of Florida.
• Home Health experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a focus on patient-centered care.
• Current CPR certification.
• Flexible availability for PRN shifts, including weekends and holidays.
• AXXESS Home Health (EMR) experience preferred.
