7 On 7 Off On Call RN

We are looking for a RN to work the "7 On, 7 Off" Schedule. Hospice Case Manager or other hospice-specific RN experience is required to be considered. This is a full time, salaried base role that will be working on call, overnight during the week and will travel throughout our 60 mile service radius around Richmond.

Job responsibilities can include but are not limited to:

• Administer skilled nursing for all ages in their place of residence.
• Conduct patient admissions.
• Conduct routine patient visits.
• Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care.
• Demonstrates accountability, flexibility, respect, and integrity.
• Submits all required paperwork and documentation within 48 hours.

Qualifications:

• Current Registered Nurse license in the state of Virginia. .
• One (1) years experience in a hospice setting required as a RN Case Manager or similar role.
• Must have a current Texas Driver's license and dependable transportation.
