Hybrid Licensed Clinical Social Worker

Job Summary: We are seeking a Licensed Clinical Social Worker to join our team! You will

provide emotional and administrative support to clients in either individual or group settings. -

HYBRID opportunities!

Responsibilities include:

● Assess nature of clients' situations by interviewing and reviewing personal history

● Develop and execute individual treatment plans

● Determine appropriate milestones to gauge client progress Communicate client updates to

all relevant parties

● Facilitate referrals to other healthcare professionals and programs Maintain accurate

client documentation

● crisis assessments/triage with appropriate interventions

● provision of other services related to wellness

● other related activities designed to provide integrated health care services

Job Requirements:

● Master's

● LCSW (New York license)

● one to two years’ experience providing clinical services in the mental health field

● Bilingual English/Spanish Speaking

● Hybrid
