We support formerly homeless adults living with co-occurring mental health and substance use disorders by providing a continuum of integrated direct care services. Position Overview
The Intake Specialist coordinates the flow of clients into clinically appropriate Baker Places, Inc. programs from external referral sources, and facilitates the smooth flow of clients between and among internal agency services. The Intake Specialist interviews clients who may be currently hospitalized, incarcerated, homeless or participating in other programs. All PRC Baker Places, Inc. services are provided from the philosophy of social rehabilitation, which integrates teaching daily living skills in a safe and healthy environment.
Primary Duties and Responsibilities
- Conducts and documents interviews with clients who may be currently hospitalized, incarcerated, homeless or participating in other programs.
- Collects and assembles all clinical information/documentation necessary for the destination program to conduct an Intake interview, ensuring documentation complies with regulatory entities. Works with providers to gather any missing eligibility information.
- Coordinates and processes completed intake materials for client placement into agency programs.
- Clinically assess written materials for appropriateness of possible placements, identifying potential risk management issues.
- Keeping up to date information via databases on client demographics, status of provider referrals, and maintain agency client census in all programs.
- Serves as liaison with Baker Residential Treatment Programs and provider referral sources.
- Demonstrates knowledge about psychiatric diagnoses, substance abuse, addiction dynamics, and HIV related issues.
- Adheres to HIPAA and all other confidentiality requirements.
- Identifies and conducts outreach services to potential community based agencies.
- Other related duties as assigned.
Minimum Qualifications
- Mandatory Mental Health Rehabilitation Specialist certification criteria:
- AA Degree and 6 years of experience in a mental health setting; at least 2 years MUST be post degree or;
- Bachelors Degree and 4 years of experience in a mental health setting or;
- Master's Degree and 2 years of experience in a mental health setting
- Preferred (3) years’ direct service experience in a dually diagnosed residential treatment program.
- An understanding of and commitment to Social Rehabilitation is required.
- Must demonstrate cultural competency while working with diverse populations on the issues of mental health, substance abuse and HIV-AIDS in a social rehabilitation treatment model.
- Ability to work collaboratively and effectively with other team members and outside agencies.
- Preferred but not required: California driver’s license, vehicle, and current auto insurance.
Physical Requirements
- Must be able to navigate several flights of stairs many times a day.
- Must be able to do extensive charting and documentation.
- Must be able to lift 20 pounds.
- Responsible to physically respond to client needs in the event of a crisis situation.
