Join Our Mission: Astor Services’ Rockland Adolescent Partial Hospitalization Program (PHP) is seeking a proactive, highly motivated, and organized Administrative Case Manager to support a dynamic, collaborative team dedicated to helping youth remain out of inpatient hospitalization. What You’ll Do:
As an Administrative Case Manager, you will play a vital role in ensuring smooth operations and exceptional care. Your responsibilities include:
- Office Support: Provide general administrative assistance to PHP staff.
- Referral Management: Schedule intakes, maintain the client census board, and track referral/admission data.
- Preadmission Coordination: Enter clients into CareLogic and complete intake consents.
- Data Management: Maintain program data required by OMH, local county, and Astor.
- Family & Community Liaison: Serve as a key contact between families, agency personnel, and community providers.
- Surveys & Screenings: Complete Social Care Network screenings and satisfaction surveys with caregivers/guardians.
- Environment of Care: Assist the Program Supervisor in managing the program environment and ensure compliance with documentation and reporting requirements.
- Special Projects: Assist with assignments identified by the PHP Supervisor or Director.

Requirements
- Education: BA/BS in Human Services or related field.
- Experience: Prior experience working with families.
- Skills: Proficiency in Microsoft Word, Excel, and PowerPoint.
- Bilingual candidates strongly preferred.
