Community Advocate / Case Manager (Hybrid)
Position 1:
Starting Pay: $25.00 / hour
Schedule: 30-36 hours per week
Position 2:
Starting Pay: $25.00 / hour
Schedule: 20-24 hours per week
Locations:
The Community Advocate will be providing services with residents who live at two programs in Minneapolis and one program in Bloomington.
Community Advocate Responsibilities:
- Establish supportive and motivational relationships with residents at three programs.
- Meet with residents and provide Integrated Community Support (ICS) Services and HSSS Services.
- Meet with residents and establish supportive and motivational relationships with clients.
- Connect clients to community resources and programs.
- Maintain client records in electronic health record system.
- Collaborate with other family members, service providers, staff members, consultants, interns and/or volunteers.
Community Advocate Requirements:
- 1-2 years of experience in behavioral health field, 30 college credits in behavioral health, or combination of experience and education required.
- Must meet the definition of Mental Health Practitioner per Department of Human Services licensing requirements.
- Experience working with clients in a residential setting preferred.
- Valid driver's license, vehicle insurance, and acceptable driving record required.
