DEPT: Early Education LOCATION: Ansonia, Milford, Seymour, Shelton, Waterbury
STATUS: F/T Regular, GRADE:
UNION: Yes
TITLE OF IMMEDIATE SUPERVISOR:
Family Support Coordinator
POSITIONS SUPERVISED: NA
BASIC PURPOSE:
To provide social services support for families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Complete recruitment applications with prospective families.
2. Complete Family Partnerships with families selected for the program.
3. Take part in community events and family activities including Family Committee Meetings
4. Report all cases of suspected child abuse to DCF.
5. Work with families to ensure they have a primary care provider.
6. Provide case management to families and maintain all appropriate documentation.
7. Visit homes of families a minimum of 3 times per year.
8. Make appropriate referrals to local social service agencies.
NON-ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Perform other related duties as required.
2. Substitute in classrooms as needed
3. Attend appropriate community service meetings outside the agency.
WORK ENVIRONMENT:
Office environment, work around children with communicable diseases, travel to client homes with varying degrees of accessibility.
Qualifications
QUALIFICATIONS:
EDUCATION: Associate’s degree in Social Work or related field, Bachelor’s degree preferred
EXPERIENCE: 1+ year in social services
SPECIAL SKILLS: Strong organizational skills, strong interpersonal skills, computer skills
PHYSICAL REQUIREMENTS: Ability to hear the conversational voice, with or without a hearing aid, the ability to see and read newsprint with or without corrective lenses, to speak and be understood under normal circumstances. Must be able to drive including getting into and out of vehicle frequently and sitting for brief periods of time, ability to climb stairs, bend and lift and reach.
