Job Summary:

As a Direct Support Professional at Futures Community Support Services, you’ll provide daily support, supervision, and guidance to individuals with disabilities living in a group home. You’ll help with personal care, medication administration, and community integration while promoting independence, dignity, and choice.

Key Responsibilities:

  • Support individuals with daily living skills such as personal care, cooking, and housekeeping

  • Implement and document Individual Service Plans (ISPs)

  • Administer medications and maintain Medication Administration Certification

  • Encourage independence and respect individual choices

  • Provide emotional support, advocacy, and community engagement

  • Complete required documentation accurately in the agency’s electronic health record system

  • Attend required trainings and staff meetings

  • Promote a positive, inclusive, and safe environment

Job Requirements:

  • Possess a valid driver's license for at least 3 years
    • License must be valid in PA or NY
  • Satisfactory DMV Background Check
  • Able to work on weekends, holidays, and during inclement weather.

Physical Requirements:

  • Lift and transfer up to 100 lbs. (with or without assistance)

  • Perform repetitive motions, bending, reaching, and wheelchair support as needed

  • Maintain awareness and quick response to ensure safety

Working Conditions:

  • Positions available for evenings, overnights, weekends, and holidays

  • Support individuals who may have co-occurring mental health needs or challenging behaviors

  • Must complete a pre-employment physical and Mantoux TB test upon hire.

Qualifications
