Physical Therapist-SS

Description

Specific duties and responsibilities:

• Complete initial eligibility evaluations, added assessments, Individual Family Service Plans (IFSP), 6 month progress reviews, annual evaluations, exit evaluations
• Write Evaluation Summary Reports and contributing information on IFSP’s and participate in all required meetings pertaining to position purpose
• Provide direct services utilizing the Family Training Model to promote appropriate child development
• Involve parents/families in techniques and activities to facilitate appropriate child development, as per Family Training Model
• Facilitate workshops, when appropriate
• Complete all required reports, session notes, and all other documentation applicable to duty
• Participate in staff meetings and/or professional development
• Fulfill job-related responsibilities in a manner reflecting professional and ethical standards of conduct that apply to their specific profession or discipline
• Perform other duties at Program Director’s request

Qualifications

Licensed Physical Therapist, State of New Jersey Department of Law and Public Safety, Division of Consumer Affairs, Board of Physical Therapy pursuant to N.J.S.A. and N.J.A.C.; AND preferably has at least one year of documented professional experience with individuals from birth to five years of age with developmental delays and disabilities.
