KeystoneCare is looking for a compassionate and dedicated LPN or RN to provide private duty nursing care to a patient in their home. The RN will be responsible for delivering skilled nursing care, ensuring the highest standards of patient safety and comfort. This role involves working closely with patient and their family to develop individualized care of the ALS patient. The ideal candidate will have a passion for helping others and be adept at providing both medical and emotional support to patients.

Current Shifts Available:

  • Monday through Sunday 7A-7p or 7P-7A

Rate:

  • LPN: $38-$40/hr
  • RN: $48-50/hr

Primary Job Responsibilities:

  • Provide personal care to patient as needed.
  • Provides skilled nursing care in accordance with physician's orders, the patient's care plan, and agency policies.
  • Administers medications and treatments as prescribed.
  • Performs patient assessments and monitors changes in patient condition.
  • Communicates effectively with patients, families, and other healthcare team members to ensure quality care.
  • Educates patients and families on care plans and safety measures.
  • Documents all patient care activities in compliance with agency policies.
  • Familiarity with Feeding Tubes, Assisted Breathing Devices (Ventilator) and Hoyer Lift.

Requirements

Job Requirements:

  • Graduate of an accredited LPN or RN program
  • Current LPN or RN license in Pennsylvania
  • Minimum of one year experience in home healthcare or a similar clinical setting
  • Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift
  • CPR certification
  • Strong communication and interpersonal skills
  • Self-motivated with the ability to work independently
  • Reliable transportation and valid Driver's License
