  • Job involves working in radiology department performing imaging procedures.
    Job Requirements
  • Must have BLS certification (American Heart Association)
  • Must have valid state licensure for Massachusetts, verified within 30 days of start
  • Must complete Fit Test prior to start
  • Must sign Trinity Code of Conduct Form
  • Must complete Trinity I9 form
  • Must sign Trinity Confidentiality of Patient & Proprietary Information & Information System Security Agreement
  • Must sign Trinity Integrity and Audit Services Form
  • Must complete Trinity Respirator Evaluation with physician signature
  • Must complete Trinity Skills Checklist (clinical contractors)
  • Must provide proof of COVID-19 vaccination or approved exemption
  • Must provide proof of MMR vaccination or immunity
  • Must have started Hepatitis B vaccination series prior to start
  • Influenza vaccine is optional
  • Must provide 2 Step PPD or IGRA test for TB screening
  • Must provide proof of Varicella vaccination or immunity
  • Must provide Physical/Health Statement within last year
  • Must complete Trinity 10 Panel Drug Screen within 30 days of start
  • Must submit Trinity Profile including resume, skills checklist, and references
  • Must complete Trinity Conflict Of Interest Disclosure Form
  • Must complete Trinity Criminal Background Check within 30 days of start
  • Must provide Trinity Education Verification
  • Must provide EPLS report within 30 days
  • Must provide GSA/OIG exclusion report within 30 days
  • Must complete Trinity Joint Commission Testing (CORE Exam) within a year
  • Must provide NSO report within 30 days
  • Must complete Trinity System Access Form one week before start
  • Must provide current Tdap vaccine or declination if no pediatric interaction
  • Must complete Trinity MA CORI if assignment in Massachusetts within 30 days
  • Must complete Trinity OFAC check within 30 days
    Schedule Information
  • Four 10-hour shifts from 1:00 PM to 11:30 PM
  • Evening shifts
  • Work every 3rd weekend
    Additional Details
  • Job requires compliance with Trinity Health policies and forms
  • Must upload all required documentation to TrinityVMS system
