Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
Influenza vaccine required during flu season (Sept 15 - April 30).
Documentation of TB skin test screening within 3 months of start date or negative QuantiFERON TB Test or T-spot within 3 months.
Physician health statement signed by MD/DO/NP/PA within 365 days of start date.
MGB Criminal Background Check within 45 days prior to start date.
MGB Resume including past 7 years of work experience.
Respirator Medical Clearance Attestation if fit test is required.
MGB References: 2 recent references, minimum 1 supervisor or above.
MGB Skills Checklist required for submission.
MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
MGB Education Verification through background check company.
MGB SSN Trace with criminal check.
MGB National Sex Offender Check within 30 days of start and updated annually.
MGB Exclusion Checks within 30 days of start and annually.
MGB New Hampshire State Checks within 30 days prior to start date.
MGB Candidate Submission Coversheet required for submission.
