Job SummaryWe are seeking a skilled and compassionate Wound Care Nurse Practitioner/Physician's Assistant to join our growing team. In this role, you will provide specialized wound management services to patients in skilled nursing and long-term care facilities. This is an excellent opportunity to make a meaningful impact on patient outcomes while working in a supportive, interdisciplinary environment.
Responsibilities- Evaluate, diagnose, and treat acute and chronic wounds, including pressure injuries, diabetic ulcers, venous stasis ulcers, and surgical wounds
- Develop and oversee individualized wound care treatment plans
Perform wound debridement and other minor procedures as clinically indicated - Monitor wound healing progress and adjust treatment plans accordingly
- Collaborate with nursing staff, advanced practice providers, and facility leadership to ensure optimal patient care
- Provide education and guidance to clinical staff on wound prevention and best practices
- Maintain accurate and timely medical documentation
- Deliver compassionate, patient-centered care with a focus on the geriatric population
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific license in good standing in Vermont
- DEA license
- Experience in wound care (long-term care or skilled nursing experience a plus)
- Strong clinical judgment and excellent communication skills
- Comfortable working with the geriatric population
- Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
