Reports directly to:  The Assistant Director and indirectly to the Director and Corporate.


Job Summary:


Provide case management services to assigned case load in compliance with the Bureau of Prisons regulations.  Manage and maintain resident files ensuring confidentiality.  Assist with ensuring residents’ success in programs.


Education and Qualifications:


The education and experience qualifications for the position of Case Manager will be a four-year degree in a social, human services, criminal justice or behavioral science program from an accredited college or university.  At a minimum, one year of experience must be working in human services, corrections, or community-based services. Work experience may be substituted for academic studies exchanging one year of work experience in a related field for one year of academic education.  Total work experience needed in lieu of the combination of education and work experience is five years.  Must have a valid driver’s license and social security card issued by the U.S. Social Security Administration, and be a United States citizen, permanent resident, or other person lawfully admitted into the United States.  Must pass federal background check.

Requirements

Job Responsibilities:


The Case Manager shall assume the following job responsibilities:


  • Conduct intake and provide an orientation for new arrival.
  • Complete IPP (Individualized Program Plan) within 10 days of resident arrival. 
  • Prepare Individual Re-Entry Plan for all assigned residents with their assigned US Probation Officers.
  • Coordinate reentry programs to ensure continuity of care for the resident and facilitate services for residents with special needs.  This coordination will include consultation with the SCTC (Supervisory Community Treatment Coordinator) for treatment that is provided under CTS (Community Treatment Services) contracts.
  • Participate in PRT (Program Review Team).
  • Provide written case notes and terminal reports.
  • Verify that residents are attending assigned treatment sessions.
  • Conduct weekly meeting with unemployed residents and complete weekly reports.
  • Conduct Progress Reviews (PR) bi-weekly with assigned residents and complete bi-weekly reports.  The PRs must have substance and indicate the resident’s progress or lack of progress. 
  • Assist residents with release plans.
  • Develop a plan to involve the resident in productive activities such as volunteer work or community service to minimize the amount of idle time.  
  • Document approved prescribed medications.
  • Network clients with community services.
  • Attend and provide input at PRT meetings and mandatory monthly staff meetings.
  • Process incident reports.
  • Filing.
  • Staff problem clients with the Program Director.
  • Work weekends on a rotational basis.
  • Available to work late nights and weekends.
  • Local travel may be required.
  • Complete all annual training required by contract.
  • Complies with federal, state and local laws and regulations.
  • Ensures rest and meal breaks are taken in accordance with California law. 
  • Reports any violation of company policy, employee handbook, Standards of Conduct, etc.…
  • Assume and complete other duties as assigned.
