About the Role:


We are looking for a detail-oriented Care Coordinator to manage service schedules, case coordination, and staff assignments. This position ensures that youth receive the best possible care and support.

Key Responsibilities:


Assign and manage 1:1 staff schedules.
Work closely with caseworkers, caregivers, and behavioral staff.
Keep accurate service records and reports.
Provide problem-solving and service coordination.

Requirements:


Bachelors degree in Human Services, Social Work, or related field.
2+ years of experience in case management or youth services.
Strong communication & organizational skills.
Familiarity with child welfare services and support programs.
