Position Summary

Join the great IN-HOUSE team at Crossview Care Center! The PRN Speech-Language Pathologist (SLP) provides evaluation, treatment, and management of communication, cognitive-linguistic, and swallowing disorders for residents in a Skilled Nursing Facility. Working on an as-needed basis, the SLP supports resident safety, functional independence, and quality of life through evidence-based therapy approaches. Coverage may include weekends, holidays, staff shortages, or high-census needs.

 

Key Responsibilities

  • Conduct comprehensive speech, language, cognitive, voice, and swallowing evaluations for SNF residents.

  • Develop and implement individualized treatment plans based on clinical assessment and resident goals.

  • Provide evidence-based interventions for dysphagia, aphasia, cognitive-linguistic deficits, voice disorders, speech intelligibility, and other communication needs.

  • Perform bedside swallowing evaluations; provide diet recommendations and collaborate with dietary and nursing teams.

  • Educate residents, families, and caregivers on communication strategies, safe-swallowing techniques, compensatory methods, and therapy goals.

  • Complete documentation and billing according to state standards, Medicare/Medicaid requirements, PDPM guidelines, and facility policy.

  • Communicate regularly with interdisciplinary team members (PT, OT, nursing, dietary, social services, physicians).

  • Participate in care conferences and IDT meetings as needed.

  • Ensure resident safety, proper equipment use, and cleanliness of therapy materials.

  • Provide reliable PRN coverage during scheduled or urgent staffing needs.

 

Qualifications

  • Master's Degree in Speech-Language Pathology from an accredited program.

  • Current state licensure as a Speech-Language Pathologist (CCC-SLP or Clinical Fellow with supervision).

  • Valid CPR/BLS certification.

  • Previous SNF or long-term care experience preferred; familiarity with geriatric populations strongly preferred.

  • Knowledge of dysphagia management and instrumental assessment interpretation (e.g., VFSS/MBSS, FEES-if applicable).

  • Strong communication, clinical reasoning, and documentation skills.

  • Ability to work independently while collaborating effectively with the rehab and nursing teams.
