FMOLHS Competency Exam with passing score of 80% or higher within year of start required.

FMOLHS Contract Annual Education Packet required for onboarding.
FMOLHS Eye Exam & Color Vision (Ishihara Color Test and Snellen Vision test) within 1 year prior to start required.
