Position Description:
Within the regulatory scope of practice, the LVN Case Manager will support the delivery of direct patient care by performing routine patient care procedures, technical tasks, and supportive functions in a clinical setting for clients enrolled in JWCH mental health programs.
Principal Responsibilities:
- Performs routine patient care functions as an LVN within the defined scope of practice, including medication management, treatment compliance, and skill-building for memory and self-sufficiency.
- Administers medications by injection as prescribed by the treating provider, verifying dosages against specified limits. Observes and reports any adverse reactions to medications as necessary.
- Conducts basic assessments (e.g., data collection) and documents pertinent patient information in accordance with JWCH policies and LVN practice standards.
- Works within a multidisciplinary team to support clients diagnosed with mental illness and co-occurring conditions through medical coordination, assessment, and accompaniment.
- Develops patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address social, emotional, and practical needs.
- Provides consultation and coordinates care for patients needing specialty mental health and/or substance use disorder services.
- Maintains cleanliness, organization, and an adequate supply of materials in the assigned work area.
- Participates in assigned staff meetings, including quality improvement sessions, case conferences, and in-service meetings.
- Conducts screenings for service requests in a timely manner, as defined by DMH Access to Care standards.
- Completes all required documentation, including assessments, progress notes, care plans, and billing records, in compliance with funding source requirements.
- Participates in structured and as-needed case conferences.
- Assists with provider education on integrated behavioral and primary care services.
- Provides ongoing case management services, including linkage and referrals to both internal programs and external resources.
- Supports clients in developing skills to manage and reduce impairment from medical and mental health conditions.
- Provides field-based care as needed for client treatment.
- Performs other duties as assigned.
Requirements:
- Current and valid California LVN License
- Current BLS certification from the American Heart Association
- At least one year of experience in a mental health setting or working with individuals with mental illness
- Strong verbal and written communication skills
- Experience working with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds
- Current and valid California Driver’s License, with current car insurance and a clean driving record
- Bilingual (English/Spanish) preferred.
