Registered Nurse Case Manager Travel

Requirements:

• Active RN license in state of assignment (or compact)
• Minimum 2 years of recent Case Management experience
• BLS certification (AHA preferred)
• Experience in discharge planning, utilization review, and care coordination
• Familiarity with EMR systems (Epic, Meditech, or Cerner preferred)

Job Summary:

As a Travel Case Manager RN, you’ll coordinate safe, effective discharge plans and evaluate the appropriateness of care across inpatient, outpatient, and post-acute settings. Collaborate with physicians, social workers, and care teams to advocate for optimal patient outcomes.

Key Responsibilities:

• Perform utilization review and monitor length of stay
• Develop individualized care plans and coordinate transitions of care
• Work with insurance providers and ensure compliance with payer guidelines
• Educate patients and families about care pathways and post-discharge support
