Job Objective: Under the direction of the Assistant Director of Pharmacy Operational Services and Director of Pharmacy Services, in close collaboration with the leadership, Transitions of Care (TOC) Pharmacist Specialist and staff of the Pharmacy Services Department, with some latitude for independent judgement and action, obtain medication histories for patients of AVMC as identified by departmental procedures. The Pharmacy Medication History Technician will also participate in transitions of care activities as assigned.
Duties and Responsibilities:
- Perform assigned duties and responsibilities that are compliant with all State, Federal, and Joint Commission laws and regulations related to the practice of pharmacy and the provision of safe medication therapy
- Obtain medication histories for patients of AVMC as identified by departmental procedures.
- Obtain medication history information from a variety of sources including, but not limited to, patients, patients’ family and/or caregivers, other pharmacies, physician’s offices, clinics, and previous electronic documentation.
- Document medication histories in the organization’s electronic health record (EHR) and clinical decision support tool.
- Communicates completion of medication history with pharmacists, nurses, and physicians as needed.
- Participate in Meds-to-Beds Program as needed
- Provide information to patients, patients’ family and/or caregivers regarding AVMC’s meds-to-beds program when appropriate
- Assist TOC Pharmacist Specialist in coordinating medication delivery efforts from the outpatient pharmacy
- Maintains an appropriate level of competence by ongoing development of knowledge and skills
- Refrains from disclosing confidential information acquired in the course of work except when authorized, unless legally obligated to do so.
- Refrains from using or appearing to use confidential information acquired in the course of work for unethical or illegal advantage either personally or through third parties.
- Assists in the appropriate orientation, training and competency assessment of pharmacy staff as it relates to medication histories and transitions of care.
- Assists in the creation and maintenance of training reference materials related to medication histories and transitions of care.
- Assists in the creation and maintenance of policies and procedures related to medication histories and transitions of care.
- Maintains professional work habits by organizing workload, setting priorities, completing assignments in a timely manner, utilizing resources appropriately, complying with department program standards and adapting well to change
- Participate in appropriate committees and meetings as necessary.
- Recognizes departures from the normal and discusses with pharmacist

Education and Experience:
Education
· High School diploma or equivalent is required
· Associates of Arts or bachelor’s degree in related field is preferred
Experience
· At least 2 years of acute care pharmacy practice as a pharmacy technician
· Experience in transitions of care preferred
Required Licensure and/or Certifications:
· Must have a current State of California Pharmacy Technician License
· Pharmacy Technician Certification Board (PTCB) certification preferred

Physical Requirements and Working Conditions:
- Combination of direct patient care and working in an office environment
- Requires ability to stand, sit or walk around the department and facility most of the day.
- Some bending, lifting less than 20 lbs. and reaching above the shoulder.
- Typing, answering phones and working around automated equipment
- Potential for exposure to dangerous or toxic medications
- Appropriate PPE when working in areas of the facility that require PPE.
