At KeystoneCare, we are committed to providing compassionate and supportive care to patients and their families at the end of life. We are seeking a dedicated RN to join our Hospice Inpatient Unit team on a per diem basis during day shifts and work every other weekends. This role involves delivering high-quality nursing care to patients in a comfortable and supportive environment. You will work collaboratively with a multidisciplinary team to ensure that each patient’s symptoms are managed effectively while honoring their wishes and dignity.

Primary Job Responsibilities:

  • Provide skilled nursing care to hospice patients in an inpatient setting.
  • Assess patient needs and determine scope and frequency of required services.
  • Implement plans of care based on individual patient and family needs, focusing on comfort, dignity, and quality of life.
  • Administer medications and manage symptoms effectively.
  • Coordinate care with other members of the hospice care team to provide comprehensive support.
  • Document patient care accurately and timely on all relevant records.
  • Maintain a professional and respectful environment, honoring patient confidentiality and rights.

Requirements

  • Graduate of an accredited school of nursing.
  • Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
  • Prior experience in hospice, palliative care, or a medical/surgical setting preferred.
  • Strong understanding of hospice philosophy and principles of end-of-life care.
  • Effective communication skills and ability to work as part of a team.
  • Demonstrated ability to manage complex patient care needs and provide emotional support to families.
  • CPR certification required.
  • Reliable transportation.
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
