Responsibilities
- Provide direct patient care in the home
- Assist with assessments and care plans
- Administer medications and treatments
- Perform wound care and patient education
- Coordinate with nurses, physicians, and care team
- Maintain accurate documentation
Requirements
- Active LPN license (Georgia)
- CPR certification
- Reliable transportation
- Strong communication and organizational skills
- Home health experience preferred
