About this job
Responsible for assisting clinical team in all aspects of the patient intake process including
reviewing referrals, establishing and maintaining positive relationships with customers and
referral sources and responding to customer requests and concerns.
DUTIES & RESPONSIBILITIES
1. Assists and participates in all daily patient referral and intake operations including clinical assessment.
2. Ensures compliance with all state and federal referral/intake regulatory requirements.
3. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Ensures seamless transition of patients to home or hospice care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
6. Builds and monitors community and customer perceptions of Pasco/SW as a high-quality provider of services.
7. Gathers, collates, and reports referral statistics including key customer referral trends.
8. Participates in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs. 
9. Maintains comprehensive working knowledge of Pasco/SW contractual relationships and ensures that patients are admitted according to contract provisions.
10. Participates in quality assessment, performance improvement teams, and activities.
11. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by
Pasco/SW Home Health and Hospice.
12. Participates in case conference meetings and clinical trainings
13. Collaborate directly with clinical supervisors and Director of clinical services

16. Hospice Intake workflow
17. Check Fax and process in coming referrals, orders
18. Maintain SOC pending list daily
19. Monitor delayed SOC orders and unsigned orders
20. Support to HH clinical team (and Hospice)
21. Clinical and Financial approval for HH referrals
22. Organize and Maintain supply closet to ensure supplies are not expired 23. Track supplies and send supply orders to HH DCS as needed.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1. Medical Assistant Certification or equivalent; relevant associate degree
2. At least three to five years of experience in health care operations.
3. Knowledge of medical terminology, EHR systems, and insurance protocols.
4. Knowledge of corporate business management, governmental regulations, and private payer practices.
5. Demonstrates good communication, organizational and problem-solving skills
